Community Support Program (CSP)
CSP is a community-based service offered to all age groups. CSP offers an array of supportive services for individuals with a history of behavioral health and/or substance use disorders that include previous hospitalizations or episodes of impairment that placed the individual at risk for hospitalization. Services are delivered by a mobile, community based, and multidisciplinary team and are designed to be flexible. Services may include transportation to and from medical appointments, care coordination among all providers, assisting the individual in developing daily living skills, coping skills, socialization and leisure skills, and accessing community resources. Services are designed to support an individual’s ability to independently access and sustain involvement with needed services. The ultimate purpose of this service is to support each member’s goals and his or her tenure in the community.

Quincy Location: Braintree, Canton, Cohasset, Hingham, Hull, Milton, Quincy, Randolph, Scituate, Weymouth.
